Year One Membership
All new patients enter membership under the Terrain Intensive Plus Tier for their first year of care. This plan allows the time, space, and resources to complete a comprehensive terrain analysis, with regular visits with the CIOFM core care team to discuss functional testing results, curate a personalized care plan, and adjust regularly as needed.

Prescription Management: Comprehensive management of prescriptions, including medication refills and adjustments.
Discounted Lab Pricing: Access to reduced rates for necessary laboratory tests.
Medical Orders and Authorizations: Assistance with imaging, procedure orders, and prior authorizations.
Collaboration with Healthcare Providers: Coordination with your oncology team and primary care provider upon your request to ensure integrated care.
Lifestyle and Nutritional Guidance: Emphasis on physician-directed detoxification, deep nutrition, and lifestyle modifications to address various health aspects, including mitochondrial health, oxidative stress, hormone balance, and gut health.

TERMS & DISCLAIMERS
All CIOFM memberships are for a 12-month minimum term, which is renewable each year. To cancel membership, you must provide written notice to CIOFM via the patient portal or in writing, attn: Dr. Jamie Waselenko. Written notice must be provided via email to jamie@ciofm.org or by US Mail (10276 Alliance Road, Blue Ash, OH 45242. Cancellations will be processed 30 days from receipt of notice.
Our monthly membership starts the day you sign up for your membership and renews that same day each month. Your member discounts and benefits renew on the 1st of each month.
Membership pricing, services, promotions, and offers are non-transferable and are only for the member’s use.
Any membership fee returned for insufficient funds, incorrect credit card numbers, or card information is considered in arrears. If a membership is considered in arrears, membership services and discounts will not be available until the balance is paid up to and including the current month's due.
Membership fees must be paid with a Visa/Mastercard/Discover card. HSA/FSA credit cards may be used.
Monthly membership fees are drafted on a 30-day rolling basis and are typically withdrawn on the 1st of each month. If a payment is declined, there is a 10-day grace period, after which an additional fee of $25 will be applied.
CIOFM members agree to abide by the CIOFM policies.
