2. Build Your Foundation
Two-Part Initial Consultation · $1,200
Visit One
A 105-minute consultation with a CIOFM Nurse Practitioner to review your health history, assess your current health, and order foundational lab work.
Between Visits
Complete your onboarding blood labs, typically $500–$800 and paid separately.
Visit Two
Return for a physical exam, review of your lab findings, and initial clinical recommendations. You will also discuss the next phase of care through Terrain Proactive membership.
Dr. Jamie provides lab review and clinical oversight throughout the program.

No. Terrain Proactive is not a cancer screening or diagnostic program, and no program can guarantee cancer prevention.
Instead, we focus on identifying and improving the many biological factors that influence health—including inflammation, immune function, metabolism, gut health, toxic burden, nutrient status, and genetics—to help create an internal environment that supports long-term wellness. This approach reflects the terrain-based philosophy that guides all of our care.

Your Initial Consultation takes place over two visits.
During the first visit, your provider will review your medical history, family history, lifestyle, and health goals before recommending foundational laboratory testing.
At your second visit, you'll review your results, discuss what they reveal about your terrain, and begin developing a personalized strategy. If Terrain Proactive is the right fit, you'll also discuss membership and next steps.

Your membership includes several advanced functional medicine assessments that help us understand how your body is functioning beneath the surface. Depending on your needs, testing may include:

Terrain Proactive includes a 12-month membership commitment. Meaningful changes in metabolism, immune function, nutrient status, and lifestyle occur over months—not weeks. This timeframe allows us to establish a strong baseline, implement personalized recommendations, reassess your progress, and make thoughtful adjustments as needed.

Your Initial Consultation Package and any recommended laboratory testing are separate from the monthly membership fee.
Your provider will review all anticipated costs with you before beginning care so there are no surprises.

Terrain Proactive is a direct-pay functional medicine program and is not billed to insurance.
This allows our providers to spend significantly more time with patients, recommend advanced testing when appropriate, and develop highly individualized care plans without the restrictions often imposed by insurance companies.

Many patients are able to use Health Savings Account (HSA) or Flexible Spending Account (FSA) funds for eligible services. Because every plan is different, we recommend checking with your HSA or FSA administrator regarding your specific coverage.

Terrain Proactive is specifically designed for individuals who do not have active cancer but want to better understand their health and reduce modifiable cancer risk.
If you have active cancer, our comprehensive Integrative Oncology program may be a more appropriate fit.

Many patients with hereditary cancer predisposition choose Terrain Proactive to better understand how nutrition, detoxification, inflammation, immune function, hormones, and lifestyle may influence their overall health alongside appropriate medical surveillance.

Many visits can be completed virtually, although some services, therapies, and in-office assessments require visits to our Blue Ash clinic.
